O.M.G.
Rapture! Unbelievable! My medical insurance company has actually told me that I have EARNED OUT! And there's a check!
(No, of course it's not a royalty check -- my lousy insurance has hit my huge deductible.) I have finally received reimbursement on name brand meds that have no generic/you buy upfront. If covered/deductible met, they pay 80%. The other reason I keep them, even with riders, is because after a certain point, they pay 100%.
I haven't seen any recent policies that offer 100%. For a little peace of mind, I put up with their pickiness. (Sometimes it doesn't matter if a doctor says: "There is no sign of X in Y." The insurance company replies, Oh, No, that code always means X -- it has no other meaning. And you have an exclusion rider on X."
The surgeon was quite offended, but I expected it, and wouldn't let him do an MRI just to confirm, until we'd examined other options.
Too bad the debt has doubled in five years -- maybe we could have arranged basic insurance for everyone, and caught a bunch of people before they became seriously in danger of dying from things that are a snap to fix up front.
